Purpose: The purpose of this survey is to quantitatively determine the content validity of the Complete Mental Status Evaluation. A test is considered content valid if items on the test appropriately measure the severity of the condition which may be experienced by the patient.

Directions: Please read each of the test items in the Complete Mental Status Evaluation. For each item, based on your clinical knowledge and experience, please indicate whether the category/item is "Irrelevant/Ineffective", "Relevant/Effective", or "Effective/Accurate" - as it describes each of the symptoms and the severity.
